Meanwhile her older brother could stack blocks into a tower at age two but still trips over his own feet at age five. What’s going on here? Nothing’s wrong with either kid. You’re just watching two completely different skill systems develop on two completely different timelines, and most parents have never heard them explained clearly.
So what actually separates fine motor from gross motor skills?
Gross motor skills are the big movements. Running, jumping, climbing stairs, throwing a ball, riding a bike, keeping your balance on one foot while pretending to be a flamingo for no reason. These rely on the large muscle groups, the ones in your legs, arms, torso and core. They’re the skills that get a kid from point A to point B, or that let them not fall over while standing still.
Fine motor skills are the small, precise movements. Picking up a Cheerio between thumb and finger, buttoning a coat, holding a pencil, cutting paper with kid scissors, zipping a backpack. These use the small muscles in the hands, fingers, and wrists, and honestly, they take a lot longer to develop because they require way more coordination between the brain and the muscles.
Here’s the thing that surprised me when I first started reading about child development: these two systems don’t develop at the same pace, and they don’t have to. A kid can be way ahead on one and totally average on the other, and that’s normal. Pediatric occupational therapists see this constantly. It’s one of the most common reasons parents show up worried when there’s actually nothing to worry about.
What development actually looks like, month by month
Milestone charts make this stuff look tidier than it actually is. Real kids don’t hit checkpoints on schedule like a train. But having rough reference points helps you know when something’s worth mentioning to a pediatrician versus when you’re just watching a kid do things in their own particular order.
Gross motor: the big stuff
- 6 months: Sitting without support, starting to push up onto hands and knees.
- 12 months: Pulling to stand, cruising along furniture, maybe a few unsteady steps.
- 18 months: Walking confidently, trying to run, climbing onto low furniture.
- 2 years: Running with better control, kicking a ball, walking up stairs with a hand held.
- 3 years: Pedaling a tricycle, jumping with both feet off the ground, standing briefly on one foot.
- 4-5 years: Hopping, skipping, catching a bounced ball, riding a bike with training wheels.
Fine motor: the small stuff
- 6 months: Raking objects with the whole hand, transferring things from one hand to the other.
- 12 months: Pincer grasp — thumb and index finger picking up small objects like puffs.
- 18 months: Stacking two or three blocks, scribbling with a crayon using a fist grip.
- 2 years: Turning book pages, using a spoon without flipping it over halfway through the trip to the mouth.
- 3 years: Drawing a circle, snipping paper with scissors, starting to use a tripod grip on a crayon.
- 4-5 years: Cutting along a line, drawing a person with a body and limbs, starting to write letters.
Notice how much later some of these fine motor skills show up compared to their gross motor counterparts? That’s not a coincidence. Fine motor control literally requires more mature brain wiring, specifically in the areas managing hand-eye coordination and small-muscle precision. A one-year-old can pull himself up to standing well before he can hold a crayon with any real control, and that’s just biology doing its thing in the order it does it.
Why parents obsess over the wrong one sometimes
I’ve noticed something that pediatric OTs mention a lot: parents tend to notice and worry about gross motor delays fast because they’re visible in a very dramatic way. A kid who isn’t walking by 15 months gets attention immediately. Everyone at the park is watching, grandma is asking questions, it’s obvious.
Fine motor delays are quieter. A kid who struggles to hold a pencil correctly at age four doesn’t look “delayed” the way a kid who can’t walk does. He just looks like he doesn’t love coloring. So this stuff slides under the radar until kindergarten, when suddenly handwriting becomes a big deal and the teacher flags it in a parent-teacher conference. By then it’s not a crisis, but it would’ve been easier to address at three than at six.
This is actually one of the most useful things I can tell you as a parent: fine motor skills are trainable in ways that are genuinely fun, and you don’t need worksheets or flashcards to do it. Some of the best fine motor practice looks nothing like “practice.”
- Playdough and putty. Squeezing, rolling, pinching. This strengthens the exact hand muscles kids need for pencil grip later, and it’s basically just play.
- Tongs and tweezers. Handing a three-year-old a pair of kid-safe tongs to move pom-poms between bowls builds the same pincer control used in handwriting.
- Lacing cards and beads. Threading string through holes forces bilateral coordination — both hands working together, one holding, one guiding.
- Cutting practice with safety scissors. Even cutting up junk mail is legitimate fine motor training. Straight lines first, curves later.
- Puzzle pieces. Grabbing and rotating small pieces to fit them into place works grip strength and spatial reasoning at the same time.
When it’s worth actually flagging something
Most kids are just doing things in their own order, on their own timeline, and it evens out. But there are specific signs that go beyond “every kid is different” and are worth mentioning to a pediatrician, according to guidance from the American Academy of Pediatrics and the CDC’s developmental milestone tracker.
- Not walking independently by 18 months.
- Unable to hold any object with a pincer grip by 18 months.
- No interest in or ability to scribble by age 2.
- Frequent tripping, falling, or bumping into things well past the age when peers have that under control.
- Extreme frustration or avoidance around fine motor tasks — refusing to color, cut, or use utensils entirely, not just disliking them.
- Loss of a skill the child previously had. This one matters more than a delay. Regression is always worth a call, no exceptions.
One important thing here: a single item on this list, on its own, usually isn’t a big deal. Kids are inconsistent, they have off days, they hyperfocus on one skill and neglect another for a while. What actually matters to a pediatrician is a pattern across multiple domains, or a skill that was there and then wasn’t. If you’re ever unsure, it costs nothing to ask at the next well visit. Early intervention services exist precisely because catching something at age two is a completely different ballgame than catching it at age seven, and pediatric OTs and PTs would honestly rather see ten kids who turn out totally fine than miss the one who needed help early.

What I keep coming back to with all this is that the two systems aren’t really in competition, even though it can feel that way when your kid is amazing at one and clumsy at the other. A kid who’s climbing furniture at eighteen months and still fumbling with a spoon isn’t behind. He’s just running two different engines at two different speeds, which is exactly how bodies are built to work. Watch for real regressions, mention real concerns to your pediatrician, and otherwise let the mess of uneven development happen. It sorts itself out far more often than the milestone charts make it seem.
